IMPORTANT ELIGIBILITY INFORMATION

Level Up is an Intensive Outpatient Treatment Program intended to supplement, not replace, a child's ongoing outpatient therapy. Participants should be prepared to engage in additional therapeutic programming, skill practice, and treatment-related activities throughout the program.

If your child is currently receiving services through Grasshopper Group, please connect with your current therapist before completing this form. Your therapist can help determine whether Level Up is appropriate for your child's individual needs and treatment plan.

Submission of an inquiry or registration form does not guarantee enrollment in the Level Up program. Program eligibility requires verification of insurance benefits and, when applicable, prior authorization/approval from the participant's insurance provider. Insurance verification typically takes 7-10 business days to complete. Participants may only be accepted into the program once all required insurance approvals have been obtained and eligibility has been confirmed. A member of our team will contact you as soon as the verification process is complete.
